6th Annual Walk 4 Autism Awareness Saturday April 30th, 2011

Want to form a team? How do you do it? Just follow these steps:
1) First, you’re now a Team Captain! Come up with a fun team name. Many team names honor
an individual with autism, such as Ben’s Buddies, Colin’s Fans, and Brady’s Bunch. Some
teams use their company name. Feel free to be creative! There is no charge to register just a
small fee if you want a event T-shirt.
2) Recruit a group of your co-workers, family members, neighbors and friends. There is no
minimum, the more, the better! There are no age or gender requirements.
3) Have each of your Team Members complete and sign a registration form (Team Member
Registration Form). Have them return it to you as soon as possible. NOTE: Parents must
provide signature on registration form for children under 18.
4) Follow the ABCs to complete the forms:
A-Team Worksheet, B-Team Member List and C- Tees Form.

All team registration materials are due by April 22nd 2011 so we can
Set aside T-shirts for your team.
6) Encourage your Team Members to collect pledges up until the event. Make copies of the
Pledge Form for each of them. Be sure your Team Members know to bring their pledge forms
and donations to the team registration area the day of the event, Saturday April 30th.
Win Team Awards for the highest amount pledge monies amount turned in on event day!
