Are there any consumers that need a "specialized model of a medical group home"?

The District is in the process of doing a needs assessment to determine if there is a genuine need for a specialized model of a medical group home, similar to an intensive behavioral group home. To do this, I will need to determine what percentage of our consumers could benefit from such a model. I am requesting the assistance of the District’s WSC’s to do so. I need to know of any consumer’s with cost plans where res hab and nursing/medical services combined total more than $ 125,000.00. If any WSC who has a consumer who fits this description would just email the name of that consumer to me at :
